Physiotherapist places the patient’s shoulder and elbow in 90 degrees of flexion and then internally rotates the shoulder. If the patient experiences pain in the region of the acromion process during this movement the test is considered as positive. Internal rotation of the shoulder in this position leads to furthermore narrowing of the subacromial space which impinges the soft tissues and gives rise to pain.
